New Patients

Welcome to Cornerstone Primary Care! We understand that choosing a physician can be a process, so we want to give you as much information upfront as possible.

What can I expect at my first visit?

When you arrive, our team will assist you with completing new patient forms and collecting copies of your insurance card and driver’s license for verification. If your insurance requires a copayment, it will be collected at that time. We accept cash, check, and credit card payments.

Your first visit with the physician will take a little longer than usual, as we take time to review your medical history and understand your healthcare needs. You’ll be asked a number of questions—some you may have answered before—but this thorough process helps us provide personalized care going forward. Unless there are no medical concerns, this initial visit will not be a physical or wellness exam.

What should I bring?
